    Eliminate the "I'm here to make the big bucks" and the:yeehaw: factor and you'll attract people who actually want to practice medicine.

    The fact that medical licenses exist, period. There's my case. When a doctor screws up sufficiently, either in severity or frequency, they don't let that doctor practice medicine. There go the big paychecks. There's the case, and it's simple enough even that your limited mental faculties should be sufficient to grasp it.

    ...eliminate the personal practical incentive for anyone to pursue a career in medicine. Altruists volunteer. And you don't find too many of them in surgical theaters. So if what you're after is fewer doctors, you've got a great idea for how to accomplish that. But we don't exactly need fewer doctors in this country.

    Ah, so you're specifically referring to surgeons, not the entire medical community. Targeted therapies are already making a lot of their functions irrelevant. They're going to have to learn to step up their game.

    MDs are already being supplemented by PAs and NPs. Again, MDs will have to learn to diversify in order to keep up.

    I'm using surgeons as an example.

    If you ever fall and break your hip, or need a tumor cut out, be sure to tell them you want targeted therapy.

    Supplemented by. Not supplanted by. And you haven't even begun to refute my point. Without a tangible personal incentive to attract them to the field, altruism alone isn't going to produce doctors.

    Okay. Then take a look at the evolution of cardiac surgery over the past decade as an example. CABG has been largely discredited. Fewer chest crackers needed to perform those. Stent implantation is coming under more and more scrutiny as its impact on morbidity and mortality is shown to be not all that great, so in many cases it's being replaced by angioplasty and, frankly, anyone who can thread a needle and inflate a balloon can perform an angioplasty. And in places not at the mercy of the FDA, stem-cell therapies are becoming the standard of care and even replacing angioplasty, so anyone who can administer an injection can treat heart disease.

    You'll still need a chest cracker for that heart transplant, but that's about it. Even vessel repair can be done by laparoscopy these days.

    You're confusing hip replacement with hip fracture. The former requires surgery; the latter doesn't. And targeted therapy is what's used to treat cancers without the need for surgical interventions.

    For the most part, I agree. The problem is with psychiatrists who just prescribe and don't treat.

    To reiterate: If some people are evil because they are born with less mental ability than others, then aren't people born with less physical ability also evil?     I had to take remedial Spanish when I got to college. :( Turns out I cheated for all 3 years of high school Spanish using something called a "google translator." And when it came time to take my required fourth year (or quarter as they call them in college) I was ill prepared.

    Also didn't help I waited until my 4th year of college to try.

    While I don't think our Shut-In would disagree that some people are stronger than other people, or that mentally retarded people are physically incapable of being geniuses, he somehow seems to think that the only thing keeping someone with an IQ of 89 from becoming the next Albert Einstein is laziness.

    Some people just aren't born that smart. They can't help it. It doesn't matter how much they wanted to be smart, they aren't able to understand complex things or retain information. That is just the nature of things. By definition, average IQ is 100. And by definition, half the people will have an IQ of below 100 and half will be above. For much of my life, I've been surrounded by smarter than average people--sometimes around people that are smarter than me, oftentimes by people that aren't quite as smart but are more determined or harder working.

    The past year or so in particular, I've worked with a lot of people that are of below average IQ. It has been a real eye-opener. Two of them can't remember something for 5 minutes and don't learn from mistakes, but both have been to college (one has a degree even). When you talk to them, they seem normal. But after a day or two, you start to realize they just aren't very bright.

    I just find it ironic, given your past, that you look down upon people for lacking ambition. What's your excuse for the past 30 years? :marathon:

    I learned a lot of that stuff in high school. And this was a school that only had a handful of computers and out of date textbooks. I guess I'm fortunate not to be in this generation.

    That said, Lil' Dicky, I'm done playing with you, but I leave you with a thought: You are pretty much everything you profess to hate in everyone else. When is the last time you picked up a book? Or are you too busy catching up on the latest superhero comic book adaptations, porn, and being small minded on discussion boards? You are hands-down, one of the pettiest, most judgmental, closed minded, and generally nasty posters on this board. I generally let it go because I realize it is all you are capable of, but don't think it isn't noticed. You aren't the clever, beloved, wisecracking misfit people once thought you were.

    I could tell you horror stories. The move towards requiring a BSN and the closure of so many diploma and associate's degree programs is one of the worst things to ever happen to nursing. I don't want to generalize and say that all, or even most, BSN's are terrible. After all, my mom has a DSN and MSN. But, you get issues with BSN grads that you simply don't get with diploma or ASN grads. For instance, my mom once hired a new grad from one of the top ten nursing programs in the United States. She scoffed and resigned during orientation when she found out she would have to do the most very basic of patient care including wiping butts. Her justification? "I have a four-year degree. I don't have to wipe butts." Yeah you do, honey. It's nursing, not accounting. How did that person ever make it through clinicals and her preceptorship without knowing she would have to wipe a butt? Her university failed her.

    You're right. It's not just the education level. The Alabama Board of Nursing has decreed that no RN will have more than 13 patients at one time and if they the maximum number, they must have two aides to help them. The problem is, working in conditions outside of those limits may cost the individual nurse her license and get the nurse sued into oblivion if they make a mistake, but the hospital simply don't care. Upper Management is going to tell Middle Management how to staff and that's the end of it. Why? Like lawyers, universities are churning out nurses just as fast as humanly possible. Lose your license? Quit? Get fired? There are 30 plus new grads waiting on your job on top of the out-of-work experienced nurses who would also love your job.
